It is important to have secure systems for recording and storing information in a health and social care setting as all information relating to clients must be kept confidential at all times except when we are legally required to disclose this [ information, for example, when a court warrant has been issued requesting certain information.
I would access guidance, information and advice about
handling information from the corporate policies I have been supplied with by ..... I would also refer to my line manager for any additional information that I may need. If there were any concerns with regards to the recording, storing or sharing of information I would report this to my line manager and agree any actions that be need to be taken in order to rectify the problem.
When record keeping I would ensure that information is concise, accurate, clear and legible, that it is reported in appropriate language and in plain English. I would ensure that records focus on the facts and not on personal opinions, confidential information about service users should be kept on their personal files and service user files should be kept in a securely locked place. ]
