The difference is, clinical advance directives only come into effect when the individual is unable to make medical decisions on their own. Here are differences

Clinical:
Do Not Resuscitate Order
Health Care Proxy
Organ and/or Tissue Donation

Administrative:
Durable Power of Attorney
Living Will
